Dementia

Did you know there are over 100 types of Dementia and not every person with Dementia is affected the same way? Getting a Dementia diagnosis can be scary and some people think it’s the end. We take a different view. Our staff are trained to support you live a purposeful life with Dementia in the comfort of your own home. We can refer you to telecare companies for equipment like pendant alarms, falls detectors etc. We are proud to say we are Dementia friendly and have supported lots of individuals live with Dementia. The key to receiving the best care with Dementia is us understanding your needs, wishes, preferences, choice and working together with you to ensure you continue to live a good life.

Alzheimer’s

This is the most common type of Dementia. Here at Evergreen Care, over the years; we have supported a lot of people with Alzheimer’s to continue to live independently at home. From support to take your medication either through prompting or administration, supporting you to carryout activities, games of your choice, taking you out, shopping etc. and we are proud of the difference this has made in our client’s lives. We are currently looking after clients 99+ in age, some couples still living in their own homes and hearing what a difference it makes for them to continue to be cared for at home is rewarding. End of Life Care

Evergreen Care’s end of life service has been created to ensure that your choice to be at home in familiar surroundings during this time is as comfortable as possible. We work with you and where you choose your family to design a holistic end of life care plan tailored to meet your needs and preferences. We work very closely with Continuing Health Care services/NHS, Hospices, GPs and other healthcare professionals to care for people at this very delicate time and ensure you are receiving the best care possible.

Some of the feedback we have received from families include, “we couldn’t have asked for a better person to be with our relative at this time”. We understand that it is not just the person that requires care, it can also be family as well who also need support and comfort and we promise to offer you just that and more.

Reablement services

Have you just come out of hospital following a lengthy admission and you need support to get back on your feet? Look no further, we are here to help. We can support you with exercises drawn up by your physiotherapist or Occupational therapist to help improve your mobility, support you to use equipment provided by the hospital including hoists and other mobility aids. We support you with tasks you are struggling with but also work with you to regain skills and confidence in undertaking tasks such as preparing your own food, making a cup of tea, making the bed etc. This can turn into on-going support or short term if you get back on your feet. We believe the best place to recover from is usually your own home and we are happy to enable you achieve that.

Physical disability

A physical disability should not stop you from living your life. Need a referral to wheelchair services or you’re not sure you have the right equipment in place to aid in your care, we are happy to refer you to relevant organisations that can support you to ensure you have the right equipment in place. Our staff receive mandatory moving and handling training in which they are trained how to use different types of equipment such as wheelchairs, full body hoists, ceiling hoists, sara -steady and many more. We work with Occupational therapists to make sure our staff are trained well to use any new equipment. Fear not, our staff are at hand to help, you are never alone.

Learning Disability

There is a lot of misunderstanding around learning disability because it is not visible. At Evergreen Care, we understand this. We would work closely with other agencies involved in your care like social workers, Doctors and other specialist teams to make sure you get the support you need. Our staff will support you with attending medical appointments, claiming benefits i.e., Filling in claim forms etc. If you are still young and you would like to step away from home and live independently, a care and support worker matched with you can support you through this journey. Trust us to enable you live your life to the full.

Stroke

A stroke can leave devastating life changing effects on people’s lives from loss of speech, reduced mobility etc. and we understand how devastating this can be to the affected person and their family. Our staff are trained to deal with the ever changing needs of our clients. If you have recently suffered a stroke and you are struggling to find the right care, we are happy to help. Neurological Conditions

These include epilepsy, Alzheimer’s disease and other types of dementia, migraines, strokes, Parkinson’s disease, MS, Cerebral palsy etc. When we first assess your needs, we will take time to understand your neurological condition, how it impacts on your life and how best you can be supported. We tailor your care and support plan to meet your specific needs. Our staff will be able to recognise symptoms that you may have and take appropriate action as need. We would work closely with specialists like MS Nurses, the stroke clinic, Alzheimer’s society etc to ensure your needs are holistically met.

Covid/Post Covid Support

We are aware that a lot of people have suffered both physical and psychological traumas as a result of COVID-19 including long COVID. As a result, we tailored our training to ensure that our staff are trained to recognise signs and symptoms of COVID 19, support you to get the medical care you need if you are not able to do this yourself, continue caring for you even when you test positive. Since the pandemic, we continue to buy additional PPE for all our staff and ensure that they are tested regularly for their safety and for your peace of mind. We follow our infection control procedures as well as government guidance on COVID-19. The majority of our staff are doubly vaccinated and boosted.

Cancer Care

Have you recently been diagnosed with cancer and feel like you can’t cope at home without any support, we are here to help. From supporting you to attend your medical appointments, accompanying you to appointments if needed, helping with referrals to Cancer support organisations etc, we have got you covered.
We would be happy to help you on this journey. One of our most recent clients we cared for with cancer described us as “outstanding” for caring for their relative that was undergoing treatment. The carer enabled the family to put aside the stress of caring for their loved one and they were able to concentrate on being there for their loved one.
