Evergreen Nursing and Rehabilitation Care Center

It is the mission of Evergreen Nursing and Rehabilitation Care Center to provide 24-hour skilled nursing care, sub-acute care and specialized services in a caring environment which fosters independence and treats each individual with love, dignity, and respect.

Evergreen provides health care for short-term and long-term patients. Individuals will be encouraged to achieve their maximum abilities and maintain their independence for as long as possible.  The caregivers at Evergreen Nursing and Rehabilitation Care Center fulfill this mission by possessing the attributes of love, joy, peace, patience, kindness, goodness, faithfulness, gentleness, and self-control when attending to an individual’s needs.

We are honored to receive the AHCA/NCAL National Quality Award Bronze Award

Bronze—Commitment to Quality—Reviewed by independent Examiners, applicants who receive the Bronze award are able to describe their mission, characteristics, and key challenges and to recognize the relationship of these factors to their ability to achieve performance improvement. The Bronze award recognizes that the provider has developed a foundation to begin a journey of continuous quality improvement, and does not necessarily correspond to any public measure of performance.

The AHCA/NCAL National Quality Award has been designed as a progressive three-step process to encourage the continuous learning and development of integrated quality systems to achieve performance excellence. Each progressive step requires a more detailed and comprehensive demonstration of quality integration and performance. The criteria for each step is based on the Baldrige Performance Excellence Program criteria.

The American Health Care Association (AHCA) is a non-profit federation of affiliated state health organizations, together representing more than 11,000 non-profit and for-profit nursing facility, assisted living, developmentally-disabled, and subacute care providers that care for approximately one million elderly and disabled individuals each day.
